Over the past decade, Libera has expanded significantly, including the opening of a new center in Morgantown that provides therapy, housing, and comprehensive wraparound services for young women. As we deepen our level of care and expand our impact, we are seeking an experienced Senior Case Manager to provide intensive case management and care coordination for participants in our SOAR Program.

This position plays an important role in Libera’s evolution from a grassroots organization into a sustainable, high-impact behavioral health provider.

Position Overview

The Senior Case Manager provides high-quality, relationship-centered case management services to high-risk young women who have experienced trauma, housing instability, system involvement (juvenile justice, child welfare), behavioral health needs, and other significant barriers to stability.

This role requires strong systems knowledge, the ability to manage complex cases, and confidence coordinating care across multiple providers and agencies. The ideal candidate brings significant experience working with high-need populations and is skilled at building trust with vulnerable youth.

SOAR (Supporting Opportunity & Accelerating Resilience) is Libera’s intensive accelerator program designed to help young women stabilize their lives, strengthen mental health, and build pathways to education and employment. This position ensures participants receive consistent, coordinated support to overcome barriers and move toward independence, resilience, and long-term well-being.

General Duties & Responsibilities

·       Conduct comprehensive needs, risk, and strengths assessments

·       Develop and monitor individualized success and accountability plans

·       Provide ongoing, relationship-centered case management using trauma-informed and strengths-based approaches

·       Coordinate services across systems, including:

o  Behavioral health providers

o  Housing and supportive living services

o  Workforce and education programs

o  Healthcare providers

o  Child welfare, probation, or other system partners (when applicable)

·       Support participants in accessing housing, employment, transportation, education, and essential resources

·       Provide crisis response, de-escalation, and safety planning as needed

·      Maintain timely, accurate, and confidential documentation

·       Recruit, screen, and onboard SOAR participants

·       Facilitate or coordinate workshops focused on life skills, career readiness, and wellness

·       Support and monitor mentor-participant relationships

·       Track participant progress and program outcomes; prepare monthly, quarterly, and annual reports

·       Collaborate closely with Libera therapists and program staff to support whole-person care

·       Build and maintain strong relationships with referral sources, community partners, employers, and training providers

·       Support research and evaluation efforts related to program outcomes

·       May provide supervision for MSW interns or support onboarding and training of new staff as the organization grows

Qualifications & Skills

·      Bachelor’s or Master’s degree in Social Work, Counseling, Psychology, or a related field

·       Active Social Work license in West Virginia (LGSW or higher preferred)

·       Minimum 5 years of experience providing case management or care coordination for high-risk youth or young adults

·       Experience managing complex cases involving multiple systems and high-acuity needs

·       Strong assessment, documentation, and care coordination skills

·       Knowledge of community resources and service systems in West Virginia

·       Strong organizational skills and ability to manage multiple priorities

·      Excellent relationship-building and communication skills

Preferred Qualifications

·       Experience working with housing instability, justice-involved youth, or young adults transitioning to independence

·       Experience in community-based or field-based case management

·       Experience supervising interns or junior staff

·       Strong understanding of trauma-informed care in rural or resource-limited settings

Additional Requirements

·      Reliable transportation and ability to travel locally

·       Travel across West Virginia for meetings or partner visits (1–4 times per month)

·       Occasional evening or weekend hours based on participant or program needs

·      Successful completion of background check, Libera Listener Training, and suicide prevention training

Pay & Benefits

·      Salary: $60,000–$65,000 per year

JOB TITLE: Operations & Outreach Assistant (Charleston)

JOB DESCRIPTION

The Operations & Outreach Assistant will support key administrative functions– freeing up program staff to focus on improving outcomes for at-risk youth. Some of these functions include volunteer management, event planning and outreach, and communication and social media management. Other tasks include office management, program reporting and expense management, participating in team meetings, and collaborating with Libera staff on projects. This is a unique opportunity to gain hands-on experience in multiple aspects of nonprofit operations and community engagement and is key to carrying out our vision to build a brighter future for youth and young women in WV.

GENERAL DUTIES & RESPONSIBILITIES

Operations & Administrative Support

● Assist with scheduling, correspondence, and administrative tasks for Charleston staff.

● Help manage digital files, documents, and organizational systems.

● Provide planning support for internal meetings and special projects.

● Work with program staff to complete program reports, manage grants, and order supplies.

Outreach & Volunteer Management

● Assist with coordinating volunteer opportunities, recruitment, onboarding, and engagement.

● Manage data input, setup, and reporting in the VMS.

● Support community outreach efforts and program awareness in Libera core counties.

● Help planning and leading community events, workshops, and group sessions.

Communication & Storytelling

● Assist with volunteer communication and monthly newsletters.

● Support communication with community partners, volunteers and event attendees.

● Support social media management, email outreach, and marketing for events/opportunities.

● Help gather stories, photos, and content from the field to amplify impact.

QUALIFICATIONS & SKILLS

● Bachelor’s Degree or minimum 5 years’ experience working in a nonprofit organization or in a similar role

● Excellent communication, interpersonal, and organizational skills

● Detail-oriented with excellent time management and project management skills

● Works well independently and in a team environment

● Ability to build strong relationships with new people, businesses, and organizations

● Enthusiastic, outgoing, and driven high-performer looking to make a difference

● Proficiency in Google Workspace; familiarity with Canva, social media platforms, or CRM tools is a plus

ADDITIONAL REQUIREMENTS

● Must be willing to travel on a weekly basis across WV

● Some weekend or evening hours may be required based on timing of events

Position Overview

The Therapist provides individual and group therapy to youth and young women experiencing complex challenges, including trauma, behavioral health concerns, system involvement, and high-risk life circumstances. Services are primarily delivered in community-based settings such as schools, juvenile detention centers, shelters, youth reporting centers, and partner organizations.

This role requires strong clinical judgment, comfort working with vulnerable populations, and the ability to build trusting relationships with youth who may have experienced significant barriers and trauma.

General Duties & Responsibilities

• Provide individual therapy to youth and young women using evidence-based, trauma-informed approaches

• Facilitate Libera model groups in schools, detention settings, shelters, and other community environments

• Conduct comprehensive biopsychosocial assessments and contribute to treatment planning

• Maintain timely, accurate, and confidential clinical documentation in accordance with ethical and professional standards

• Provide crisis intervention and safety planning as needed

• Support care coordination by connecting clients to appropriate community resources and services

• Advocate for clients within systems and with community partners

• Collaborate with Libera staff and external providers to support whole-person care

• Participate in program evaluation and quality improvement efforts

• Support community capacity-building by helping deliver Libera Listening trainings and offering guidance to volunteers working with youth

• Engage in ongoing professional development and clinical best practices

Required Qualifications & Skills

• Master’s degree in Social Work, Counseling, or related behavioral health field from an accredited program

• Active West Virginia licensure as an LPC, LGSW, LCSW, or equivalent

• Minimum 2 years of post-graduate clinical experience providing therapy to youth or young adults

• Experience working with high-risk, trauma-impacted, or system-involved youth

• Demonstrated ability to provide both individual and group therapy

• Strong understanding of trauma-informed, strengths-based, and community-based care models

• Comfort working in nontraditional settings and with clients experiencing complex needs

Preferred Qualifications & Skills

• Experience with DBT-informed interventions or certification in DBT

• Experience in juvenile justice, child welfare, school-based mental health, or community behavioral health

• Experience providing crisis intervention

Additional Requirements

• Ability to travel regularly within the region and occasionally across the state

• Willingness to work occasional evenings or weekends based on program needs
