This is an agreement between the youth and the DHHR. It DOES NOT place the youth back into "custody" of the state.  The youth has certain responsibilities to uphold in working towards meeting the goals and objectives of their transition plan and in working towards becoming semi-independent and self-sufficient. The Department has responsibilities to provide the youth with assistance in meeting those goals and objectives. If a youth desires to continue to receive foster care services after receiving an explanation of the SS-FC-18, they must agree to sign a Continued Foster Care Services Agreement (SS-FC-18). Guides those in the foster care system to create a plan when they are within 90 days of leaving foster care. It includes sections on education, employment, housing, mentoring, family and other connections, and health insurance.

Youth may receive assistance to complete unique programs designed to help them achieve independence. Youth ages 16-23 transitioning from foster care or experiencing homelessness. Transitional Living Program helps young adults aged 16-22 transition to adulthood by providing them assistance with housing, life skills training, educational support, and vocational assistance.

The Foster Care Ombudsman provides assistance and resources to people who are served or affected by the child welfare system. People contact the Foster Care Ombudsman when they have a complaint or concern and a) don’t know where to go with it, b) have tried to address it through existing systems but were dissatisfied, or c) are afraid. FosterClub creates and connects young people from foster care with unique tools and programs.

The Public Market is a project of Grow Ohio Valley, a non-profit food hub in Wheeling, WV. They offer a SNAP stretch program, that allows those with SNAP benefits to buy produce at 50% off.
